It's a bug present in all versions. It's especially glaring if you strip and look in a mirror. You'll be naked in the mirror, but look down and you have underwear on. Also possibly related is sometimes equipping new stuff doesnt update on your first person model until you unequip and reequip them.Cyberpunk 2077.
