Recently my 2000 ZX3 5 speed had an issue with overheating ONLY while driving about 60mph or higher for longer than 10-15 minutes. It did this while outside temps were only 35 degrees. Your heater in the car works of engine coolant temperature, so it assists the radiator with cooling the engine. Second, as domnatr6 said, pop the hood, start the car, and crank up the AC. The engine fan should run constantly. You should not be getting overheating at 220 degrees since that is near normal op temp. Sometimes, but not always, coolant will leak into engine oil with a bad head gasket and turn it milky. Bad thermostat does not seem likely if engine temp is only reaching 220.
